As a Physical Therapist in the dynamic Home Health Therapy unit, you’ll gain invaluable experience with a diverse patient base, from orthopedic and joint replacement cases to geriatric patients managing chronic conditions such as CHF and COPD. Working alongside experienced clinicians and care teams, you’ll expand your expertise and adaptability, all while enjoying a schedule that supports real work-life balance—no weekends or holidays required.
Here, you’ll be hands-on with patients, conducting SOC visits (particularly orthopedics when nursing is not ordered), managing an average of five patient visits per day, and utilizing the efficient HomeCare HomeBase (HCHB) EMR system—streamlined on a provided tablet for ease on the go. Mileage is reimbursed, and as the therapy team grows, your travel within the friendly service area will become even more manageable.
Key Qualifications:
- Active PT license
- BLS certification required
- Home Health experience is required (open to travelers with less than 2 years’ overall experience)
- Comfort managing 5 patient visits per day
- Experience with orthopedic/geriatric patient populations preferred
- Familiarity with HomeCare HomeBase (HCHB) is a plus
Responsibilities:
- Deliver in-home physical therapy services, complete SOC visits as needed
- Provide patient care, develop and follow comprehensive care plans
- Collaborate with intake RNs, managers, and nurse aides
- Accurate, timely EMR documentation using provided tablet
- Serve the Tehama County area with a flexible, patient-centered approach
