Qualifications:

• Valid and active Physical Therapy license (in good standing) for the state of Connecticut
• Experience in hospital or outpatient therapy preferred
• Familiarity with safe patient handling, infection control, and workplace safety protocols
• Commitment to professional codes of conduct, ethical patient care, and regulatory compliance
• Exceptional communication and organizational skills

Key Responsibilities:

• Conduct comprehensive patient evaluations, develop and adjust individualized treatment plans based on patient goals and physician direction
• Delegate and direct discipline-specific therapy services, ensuring high-quality standards
• Facilitate smooth transitions for patients back to their community activities through functional treatment programming
• Foster patient and employee safety, utilizing provided equipment and adhering to protocols for handling, infection control, and environment safety
• Accurately document all medical records in a timely manner following federal, state, and clinic guidelines
• Mentor students, educate patients and families, and engage in ongoing professional development
• Participate in continuous quality improvement efforts, offering constructive feedback and implementing action plans
