Required Experience & Qualifications:

• Active California Physical Therapist License
• Minimum one year of Home Health experience
• Strong communication and organizational skills
• Proficiency in documentation and timely reporting

Responsibilities:

• Prioritize and complete start-of-care visits, with next-day mandatory reporting
• Supervise Physical Therapy Assistants to ensure optimal patient outcomes
• Submit clinical notes and documentation within 48 hours of each visit
• Maintain clear, timely communication with supervisors, intake coordinators, and office staff
• Adapt to caseload expansion as territory requirements evolve
• Responsiveness during standard business hours
