As a Physical Therapist working with a leading regional care provider, you’ll gain professional growth through exposure to dynamic home health environments across multiple service territories, including San Jose, Santa Clara, Sunnyvale, and neighboring communities. Collaborate with multidisciplinary teams, shape patient recovery at critical milestones, and strengthen your resume by adapting to diverse workplace practices and leadership styles.

Schedule/Hours:

  • Hours approved per visit; assignments may require weekend and occasional travel outside assigned territory
  • Productivity goal: 27 points per week (specific visit types equate to points/hours)
  • 36-hour weekly guarantee may apply when productivity goals and assignment requirements are met

Key Qualifications:

  • Valid California Physical Therapy License
  • BLS Certification (current)
  • Active driver’s license, auto insurance, and auto registration
  • Annual TB test, MMR, Varicella, HepB, Tdap (within 10 years), Flu (annual), drug screen, and recent physical
  • Willingness to cover a wide territory and provide weekend care if necessary
  • Prior experience in home health and managing a variety of patient needs is a plus

Role Responsibilities:

  • Deliver comprehensive evaluations and develop outcome-focused care plans
  • Perform OASIS assessments (SoC, ReCertification, RoC, discharge, and follow-ups)
  • Maintain accurate, timely patient records and documentation
  • Communicate findings and collaborate with interdisciplinary care teams and families
  • Travel within assigned counties as needed to meet caseload expectations
