Travel Physical Therapist - Fremont, CA

Schedule/Hours

• Minimum 36 hours/week guaranteed for productivity-compliant candidates
• Weekend coverage may be required
• Flexibility to occasionally travel outside the assigned area

Qualifications

• Current and active Physical Therapy license in CA
• BLS certification required
• Valid driver’s license, registration, and auto insurance
• Up-to-date TB test (annual), drug screen, physical, and immunizations (MMR, Varicella, HepB, TDap, Flu)
• Experience with hospital or home health therapy preferred

Responsibilities

• Manage a physical therapy caseload, including OASIS assessments (SoC, RoC, Recert, Discharge) and routine visits
• Deliver evidence-based therapy to patients in their homes or community settings
• Provide accurate visit and documentation time according to established productivity standards
• Coordinate care and communicate with physicians, patients, and caregivers
• Ensure compliance with agency policies and California regulations
