Physical Therapist - Gilbert, AZ

Providing expert physical therapy services in an outpatient hospital setting, this role is responsible for evaluating patients, developing tailored treatment plans, and delivering discipline-specific therapy to support effective return to daily community activities. You will play a critical role in patient care, education, and therapy program development while upholding high professional and ethical standards.

Responsibilities:

• Conduct comprehensive evaluations of assigned patients and develop individualized treatment plans aligned with patient goals and physician directives.
• Delegate and coordinate discipline-specific therapy services ensuring cohesive and effective care delivery.
• Design functional treatment programs that promote patient independence and community re-entry.
• Educate patients, families, students, and coworkers on therapy processes and health management.
• Maintain detailed, timely, and accurate medical documentation in compliance with regulatory and organizational policies.
• Participate in student training programs, providing supervision, learning objectives, and evaluations.
• Engage in ongoing professional development and comply with continuing education and licensure requirements.
• Promote and ensure patient and employee safety by practicing and advocating safety protocols consistently.

Qualifications/Desired Experience:

• Licensed Physical Therapist with current credentials in good standing.
• Proficiency in outpatient physical therapy evaluations and treatment plan development.

• Experience in documentation compliant with federal, state, and clinic-specific regulations.
