Schedule:
- Preference for Sunday–Thursday or Tuesday–Saturday schedule
- May require occasional floating to a nearby sister facility within walking distance
Qualifications:
- Active California Physical Therapist license
- Previous skilled nursing facility (SNF) experience preferred; mentorship available for those with 0–1 year of experience
- Proficient documentation and time-management skills
- Flexible, collaborative, and eager to adapt to changing needs
Responsibilities:
- Evaluate and treat patients in a skilled nursing setting, focusing on individualized care plans
- Develop and implement effective treatment programs aimed at increasing patient strength, mobility, and independence
- Collaborate closely with an interdisciplinary care team to ensure comprehensive patient care
- Complete accurate and timely documentation in Net Health in accordance with facility standards
- Maintain a professional and caring approach to each patient’s journey
