Schedule:

• Preference for Sunday–Thursday or Tuesday–Saturday schedule
• May require occasional floating to a nearby sister facility within walking distance

Qualifications:

• Active California Physical Therapist license
• Previous skilled nursing facility (SNF) experience preferred; mentorship available for those with 0–1 year of experience
• Proficient documentation and time-management skills
• Flexible, collaborative, and eager to adapt to changing needs

Responsibilities:

• Evaluate and treat patients in a skilled nursing setting, focusing on individualized care plans
• Develop and implement effective treatment programs aimed at increasing patient strength, mobility, and independence
• Collaborate closely with an interdisciplinary care team to ensure comprehensive patient care
• Complete accurate and timely documentation in Net Health in accordance with facility standards
• Maintain a professional and caring approach to each patient’s journey
