Schedule (Preferred):
- Sunday through Thursday or
- Tuesday through Saturday
- May require floating to a nearby sister facility (within walking distance)
Qualifications:
- Current and active Physical Therapist license in California
- Skilled nursing facility experience preferred; mentorship available for those with 0-1 years of experience
- Demonstrated proficiency in documentation and time management
- Adaptable and collaborative with a patient-centered approach
Key Responsibilities:
- Conduct comprehensive evaluations of patients admitted to the skilled nursing facility
- Design, implement, and adjust individualized treatment programs to support mobility and daily functioning
- Collaborate closely with the interdisciplinary care team to deliver coordinated care and maximize patient outcomes
- Complete timely and accurate documentation using Net Health software within facility guidelines
- Maintain open communication with patients, families, and colleagues, ensuring the highest quality of rehabilitative care
