Physical Therapist in Gilroy, CA

Schedule (Preferred):

• Sunday through Thursday or
• Tuesday through Saturday
• May require floating to a nearby sister facility (within walking distance)

Qualifications:

• Current and active Physical Therapist license in California
• Skilled nursing facility experience preferred; mentorship available for those with 0-1 years of experience
• Demonstrated proficiency in documentation and time management
• Adaptable and collaborative with a patient-centered approach

Key Responsibilities:

• Conduct comprehensive evaluations of patients admitted to the skilled nursing facility
• Design, implement, and adjust individualized treatment programs to support mobility and daily functioning
• Collaborate closely with the interdisciplinary care team to deliver coordinated care and maximize patient outcomes
• Complete timely and accurate documentation using Net Health software within facility guidelines
• Maintain open communication with patients, families, and colleagues, ensuring the highest quality of rehabilitative care
