Qualifications:

• Current license as a Physical Therapist
• Demonstrated experience in a hospital or acute care setting
• Proficient with EPIC electronic medical records system
• Strong interpersonal and communication skills
• Ability to contribute to multidisciplinary care planning and execution
• CPR certification and relevant continuing education preferred

Key Responsibilities:

• Conduct thorough patient assessments to determine appropriate therapy interventions
• Develop and implement individualized treatment plans to meet patient needs and functional goals
• Collaborate with physicians, nurses, and other care team members to optimize patient outcomes
• Document evaluations, plans, and progress notes accurately using EPIC
• Educate patients and families on therapy programs, home exercises, and safe mobility techniques
• Monitor, evaluate, and adjust treatment plans in response to patient progress
• Maintain compliance with institutional and regulatory standards for therapy documentation and care delivery
