Step into a rewarding opportunity as a Physical Therapist within a leading hospital-based home health service, supporting patients in California. This contract role focuses on delivering comprehensive therapy in a home-based care setting, working closely with OT and Speech teams to provide high-quality rehabilitation. You'll serve a dedicated geographic area, ensuring patients receive personalized, effective therapy in the comfort of their own homes.
Schedule/Hours:
- Flexible scheduling, majority of hours are Monday through Friday, 8am-5pm
- Weekend rotation: every third weekend
- Minor holidays required; major holidays only as needed
Requirements:
- Current and active California PT license
- Minimum of two years of full-time therapy experience
- At least one year of experience in PT, OT, or Speech therapy
- Previous home health setting experience preferred, but not required
- Proficiency with Oasis documentation
- Basic Life Support (BLS) certification is required
- Familiarity with Cerner EMR
- Professional attire with a white lab coat (no jeans)
Duties & Responsibilities:
- Provide physical therapy services to patients within their homes, addressing a range of conditions such as orthopedic issues, post-stroke care, and spinal injuries
- Collaborate with occupational and speech therapists to coordinate patient care
- Complete thorough and compliant documentation using the Oasis platform
- Attend a 1.5 to 2-week orientation specific to the assigned area/region
- Educate patients and family members on exercises, care plans, and progress
- Adapt treatment plans based on patient progress and interdisciplinary input
