Physical Therapist in Guadalupe, CA

This opportunity is for an experienced Physical Therapist to provide home health care services in the Guadalupe, CA area. The role involves delivering high-quality therapy to patients with a variety of diagnoses, including orthopedic issues, strokes, joint concerns, and spinal injuries. Care will be administered within an assigned geographic region, primarily during flexible weekday hours. Key Responsibilities:

• Conduct thorough patient assessments and develop individualized therapy plans
• Provide physical therapy treatment primarily for orthopedic and neurological conditions
• Complete government-mandated OASIS documentation accurately and on time
• Collaborate with other healthcare providers including Rehab Aides, OTAs, and PTAs
• Maintain professional business attire with white lab coat during visits
• Adhere to scheduling requirements, including weekend rotations every third weekend and occasional major holidays as needed
• Participate in orientation and ongoing training as required

Desired Qualifications:

• Active Physical Therapy license in California
• Minimum 2 years of relevant experience, preferably including home health care
• Current BLS (Basic Life Support) certification
• Experience with OASIS documentation and Cerner EMR preferred
• Familiarity with outpatient and inpatient therapy not required but beneficial
• Strong communication skills and ability to work within assigned geographic regions

Schedule:

• Majority of hours Monday through Friday, 8 AM to 5 PM, with flexible scheduling
• Weekend requirement every 3rd weekend; minor holiday coverage required
