Key Responsibilities:
- Provide comprehensive physical therapy evaluations and evidence-based treatments under physician orders
- Collaborate with multidisciplinary teams, incorporating resident and caregiver input into personalized therapy plans
- Document all assessments, treatments, and progress in the Electronic Medical Record in clear, accessible language
- Educate residents, families, and staff on therapy plans, assistive devices, and rehabilitation principles
- Mentor new staff and support development of programs and service enhancements
- Maintain compliance with Medicare, Medicaid, and private payer regulations
- Uphold clinical and workplace standards, ensuring a clean, safe, and organized therapy environment
- Supervise and collaborate with assistant-level staff as appropriate
Required Qualifications:
- Bachelor’s degree in Physical Therapy (Master’s or Doctorate preferred)
- Active Physical Therapy license in MA
- Basic Life Support (BLS) certification preferred
- Prior experience in long-term care/skilled nursing desirable
- Exceptional communication, documentation, and team collaboration abilities
