Key Responsibilities:

• Provide comprehensive physical therapy evaluations and evidence-based treatments under physician orders
• Collaborate with multidisciplinary teams, incorporating resident and caregiver input into personalized therapy plans
• Document all assessments, treatments, and progress in the Electronic Medical Record in clear, accessible language
• Educate residents, families, and staff on therapy plans, assistive devices, and rehabilitation principles
• Mentor new staff and support development of programs and service enhancements
• Maintain compliance with Medicare, Medicaid, and private payer regulations
• Uphold clinical and workplace standards, ensuring a clean, safe, and organized therapy environment
• Supervise and collaborate with assistant-level staff as appropriate

Required Qualifications:

• Bachelor’s degree in Physical Therapy (Master’s or Doctorate preferred)
• Active Physical Therapy license in MA
• Basic Life Support (BLS) certification preferred
• Prior experience in long-term care/skilled nursing desirable
• Exceptional communication, documentation, and team collaboration abilities
