This assignment immerses you in a variety of home health environments, expanding your clinical acumen by treating patients in multiple counties. Work closely with diverse patient populations, collaborate with experienced interdisciplinary teams, and learn a wide array of documentation and productivity systems. Expect valuable exposure to different leadership strategies, workflow practices, and approaches to care—enhancing your adaptability and resilience, while adding depth to your professional experience.

Schedule/Hours:

  • 36-hour weekly guarantee (for applicable assignments)
  • Weekend coverage may be required
  • Flexibility for travel outside the core area as needed

Responsibilities:

  • Deliver high-quality physical therapy services to patients in home health settings, following physician orders and best practices
  • Perform comprehensive assessments, document care episodes (OASIS and routine visits), develop individualized care plans, and manage ongoing re-evaluations
  • Complete timely documentation, drive between patient homes, and coordinate care with physicians and healthcare teams
  • Remain adaptable for floating assignments, weekend shifts, and coverage beyond the core territory

Qualifications:

  • Current California Physical Therapy license
  • BLS certification
  • Valid driver’s license, auto insurance, and vehicle registration
  • Up-to-date immunizations (TB, MMR, Varicella, HepB, TDap, Flu)
  • Ability to work independently and manage a flexible schedule
  • Comfort serving patients with varying diagnoses, including Covid+ and PUI
