This assignment immerses you in a variety of home health environments, expanding your clinical acumen by treating patients in multiple counties. Work closely with diverse patient populations, collaborate with experienced interdisciplinary teams, and learn a wide array of documentation and productivity systems. Schedule/Hours:

• 36-hour weekly guarantee (for applicable assignments)
• Weekend coverage may be required
• Flexibility for travel outside the core area as needed

Responsibilities:

• Deliver high-quality physical therapy services to patients in home health settings, following physician orders and best practices
• Perform comprehensive assessments, document care episodes (OASIS and routine visits), develop individualized care plans, and manage ongoing re-evaluations
• Complete timely documentation, drive between patient homes, and coordinate care with physicians and healthcare teams
• Remain adaptable for floating assignments, weekend shifts, and coverage beyond the core territory

Qualifications:

• Current California Physical Therapy license
• BLS certification
• Valid driver’s license, auto insurance, and vehicle registration
• Up-to-date immunizations (TB, MMR, Varicella, HepB, TDap, Flu)
• Ability to work independently and manage a flexible schedule
• Comfort serving patients with varying diagnoses, including Covid+ and PUI
