Schedule/Hours:
- Assignments may involve travel throughout multiple counties.
- Occasional weekend coverage may be required.
Desired Qualifications:
- Active California Physical Therapist license
- Current BLS certification
- Valid driver’s license, auto insurance, and vehicle registration
- Annual TB test, current immunizations (MMR, Varicella, HepB, Tdap within 10 years, annual flu)
- Ability to work with Covid+ and Covid PUI patients as needed
Key Responsibilities:
- Conduct in-home physical therapy evaluations and treatments in a variety of locations (homes, facilities)
- Complete all required documentation, including OASIS assessments and care notes
- Communicate effectively with patients, caregivers, and care teams
- Maintain compliance with all policies, standards, and travel area assignments
- Flexibility to expand outside core travel area for full caseload and to meet productivity expectations
