Schedule/Hours:

• Assignments may involve travel throughout multiple counties.
• Occasional weekend coverage may be required.

Desired Qualifications:

• Active California Physical Therapist license
• Current BLS certification
• Valid driver’s license, auto insurance, and vehicle registration
• Annual TB test, current immunizations (MMR, Varicella, HepB, Tdap within 10 years, annual flu)
• Ability to work with Covid+ and Covid PUI patients as needed

Key Responsibilities:

• Conduct in-home physical therapy evaluations and treatments in a variety of locations (homes, facilities)
• Complete all required documentation, including OASIS assessments and care notes
• Communicate effectively with patients, caregivers, and care teams
• Maintain compliance with all policies, standards, and travel area assignments
• Flexibility to expand outside core travel area for full caseload and to meet productivity expectations
