Schedule Options:

  • Preference for Sunday through Thursday or Tuesday through Saturday shifts.

Qualifications:

  • Current, active Physical Therapist license in California.
  • Previous skilled nursing facility experience highly valued; candidates with 0-1 years' experience are encouraged to apply and will receive mentorship.
  • Expertise in therapy assessment and treatment within a hospital or skilled nursing setting.
  • Excellent documentation skills and the ability to utilize Net Health or similar electronic medical record systems.
  • Strong time management, flexibility, and a collaborative spirit.
  • Willingness to float to a nearby partner facility as needed (all locations within easy walking distance).

Key Responsibilities:

  • Evaluate, plan, and implement individualized treatment programs for patients in a skilled nursing environment.
  • Collaborate closely with an interdisciplinary healthcare team to optimize patient outcomes.
  • Maintain up-to-date patient treatment documentation in accordance with facility standards and regulatory requirements.
  • Contribute to the creation of a supportive, empathetic, and goal-oriented therapy environment.
