Deliver impactful care as a Physical Therapist supporting adult patients with chronic diseases and orthopedic needs in a mobile home health role. This contract position requires travel between multiple cities within the region, with an average of 60-80 miles driven per day. You'll conduct six patient visits daily, using modern technology and thorough documentation tools provided for your work. Qualifications:

  • Active Physical Therapist licensure in the state
  • A minimum of one year of recent home health experience required
  • Current BLS certification
  • Valid driver’s license and proof of insurance
  • Confidence and experience working independently in home health settings
  • Comfortable with mobile documentation and EPIC Rover (view only)

Key Responsibilities:

  • Deliver evidence-based PT interventions to adult patients across a diverse caseload
  • Manage a daily schedule of six visits, coordinating with patients the day before each visit
  • Travel between assigned service areas, with mileage reimbursed weekly
  • Complete timely and accurate clinical documentation using Home Care Home Base and facility-assigned technology
  • Collaborate with schedulers, clinical managers, and departmental leadership for seamless patient care
  • Uphold a team-oriented, supportive environment with a commitment to high-quality outcomes
