Deliver exceptional patient care while exploring a dynamic home health therapy opportunity supporting adults with a variety of chronic and post-operative diagnoses. This contract role focuses on hospital-based therapy services where you will travel throughout multiple surrounding areas, managing your own patient schedule and ensuring continuity of care in a supportive, collaborative department culture.
Schedule:
- No weekend rotation
- No on-call responsibilities
- No major holiday coverage
- Schedule cycle renews every 13 weeks
Qualifications:
- Minimum of 1 year experience in home health therapy (PT required)
- Current BLS certification
- Active state licensure for Physical Therapy
- Valid driver’s license and proof of insurance
- Comfort with adult patients managing chronic diseases, orthopedic injuries, and post-surgical recovery
- Willingness and ability to travel approximately 60–80 miles per day; mileage reimbursement provided
- Seasoned home health therapists welcomed, including first-time travelers with relevant experience
Key Responsibilities:
- Provide comprehensive patient assessments and therapy services in various home settings
- Manage a daily caseload of six home visits, scheduling appointments directly with patients a day in advance
- Maintain accurate electronic documentation using EPIC Rover, Focura, and Home Care Home Base on company-provided Samsung Note 9 device
- Collaborate with a robust interdisciplinary team, including departmental leaders, clinical managers, supervisors, and peers
- Upload discharge instructions and critical documentation as needed
- Adhere to department requirements: scrubs in grey, free on-site parking available
