Schedule/Hours

Key Responsibilities

• Conduct thorough evaluations and develop customized care plans with functional long- and short-term goals
• Administer therapeutic interventions in collaboration with physicians and caregivers
• Monitor progress, adjust treatment as needed, and document all services meticulously in the EMR system
• Educate patients and families on therapy plans, use of assistive devices, and discharge needs
• Collaborate with assistants and interdisciplinary teams; provide mentorship to new staff
• Maintain compliance with Medicare and insurance regulations
• Support department cleanliness and adhere to infection control procedures

Qualifications

• Bachelor’s degree in Physical Therapy required (Master’s or Doctorate preferred)
• Current PT license in MA
• Basic Life Support (BLS) preferred
• Prior experience in long-term care or skilled nursing facilities is a plus
• Strong communication, analytical, and time management abilities
• Compassionate, team-oriented, and skilled in documentation
