Schedule/Hours:

Qualifications & Desired Experience:
- Degree in Physical Therapy from an accredited program
- Active PT license in the state of Colorado (or eligibility to obtain)
- Experience with outpatient therapy preferred
- Comprehensive knowledge of therapy evaluation, plan development, and implementation
- Commitment to upholding high standards of patient confidentiality and professional conduct
- Strong communication and clinical documentation skills

Key Responsibilities:
- Perform comprehensive patient evaluations; develop, implement, and modify individualized plans aligned with physician direction and patient-centered goals
- Delegate and direct discipline-specific therapy services for assigned patients
- Design functional programs promoting safe and effective return to daily activities
- Provide patient, family, and team education to support the care process
- Maintain accurate, timely medical record documentation in compliance with regulatory standards
- Uphold rigorous safety standards regarding patient handling, infection control, and clinic environment
- Participate in professional growth activities, adhering to state licensure and ethical codes
- Contribute to continuous improvement initiatives through feedback and participation in quality programs
