Responsibilities:
- Conduct thorough evaluations for assigned outpatient cases and develop, implement, and modify individual treatment plans
- Set and track patient-centered goals toward effective community reentry
- Delegate and supervise therapy services for assigned patients
- Maintain clear, concise, and timely clinical documentation, compliant with all relevant policies and regulations
- Serve as an educator for patients, families, students, and colleagues

Qualifications:
- Valid Physical Therapy licensure in the state (or eligibility to obtain before starting)
