Schedule/Hours:

Qualifications:
- Bachelor’s degree required; Master’s or Doctorate in Physical Therapy preferred
- Active Physical Therapist license in the state of Massachusetts
- Prior experience in long-term care or skilled nursing preferred
- Basic Life Support (BLS) certification preferred
- Strong communication, customer service, and time management skills
- Demonstrated ability to work collaboratively with interdisciplinary teams

Key Responsibilities:
- Evaluate and develop personalized therapeutic plans for residents
- Lead and supervise assistant-level staff as appropriate
- Collaborate with residents, families, and healthcare team members to maximize patient independence and outcomes
- Ensure timely, accurate documentation in Electronic Medical Records
- Provide education for residents and caregivers, including safe assistive device use
- Stay current with professional standards, participate in department initiatives, and mentor new staff
