Schedule Options:

  • Sunday through Thursday
  • Tuesday through Saturday

What You’ll Need:

  • Active California Physical Therapist (PT) License
  • Experience in a skilled nursing facility (SNF) preferred; mentorship provided for candidates with 0-1 years’ experience
  • Excellent documentation and time-management skills
  • Flexible, positive, and collaborative attitude
  • Willingness to float between facilities, both located within walking distance

Key Responsibilities:

  • Assess and treat residents in a skilled nursing facility setting
  • Design and execute individualized treatment plans to meet mobility and rehabilitation goals
  • Collaborate with an interdisciplinary care team to ensure the best patient outcomes
  • Accurately complete documentation within Net Health according to facility standards
