Key Responsibilities

• Evaluate residents and develop personalized therapy plans to maximize independence and functional abilities.
• Perform therapeutic procedures under a physician’s or healthcare provider’s supervision while adhering to safety standards and mandatory precautions.
• Collaborate closely with residents, caregivers, and the healthcare team to establish and progress care goals.
• Document all treatments and clinical interactions in the Electronic Medical Record per established protocols.
• Recommend and instruct on proper use of assistive devices.
• Deliver effective education to residents and caregivers, recording outcomes.
• Participate in care planning meetings, provide in-service training, and support program development within the rehabilitation department.
• Adhere to Medicare and other reimbursement requirements in all clinical documentation.
• Supervise and mentor PT assistants and new staff as needed.

Qualifications

• Bachelor’s degree in Physical Therapy required; Master’s or Doctorate preferred.
• Active Physical Therapist license in Massachusetts.
• Prior experience in long-term care or skilled nursing is beneficial.
• Basic Life Support (BLS) certification preferred.
• Strong communication, documentation, analytical, and time management skills.
• Ability to lift up to 50 pounds, with consideration for safe lifting protocols.
