Physical Therapist - Monterey, California

Schedule/Hours:

• Preference for either Sunday–Thursday or Tuesday–Saturday schedules

Qualifications:

• Active California Physical Therapist (PT) license
• Experience in skilled nursing facilities preferred; mentorship and support available for therapists with 0–1 years’ experience
• Strong skills in patient documentation and time management
• Able to work collaboratively and adapt in a dynamic care environment
• Comfortable with flexible assignments; occasional float to a nearby sister facility (within walking distance) may be required

Key Responsibilities:

• Evaluate patients and develop individualized treatment plans based on current best practices
• Administer physical therapy interventions focused on improving mobility, balance, and overall functional independence in post-acute and outpatient settings
• Collaborate proactively with an interdisciplinary care team, including physicians, nursing staff, and other therapy professionals, to achieve optimal patient outcomes
• Maintain accurate, timely clinical documentation using Net Health in accordance with facility standards
• Participate in periodic team meetings, sharing insights and strategies for patient care advancement
