Schedule:
- Preferred shifts are Sunday–Thursday or Tuesday–Saturday.
Qualifications:
- Current and valid Physical Therapist license in California
- Experience in skilled nursing or rehabilitation settings preferred; mentorship available for candidates with less than one year of experience
- Strong documentation and organizational abilities
- Ability to work flexibly in a team environment
- Willingness to float to a nearby sister facility if needed
Responsibilities:
- Assess, plan, and implement treatment interventions for patients in the skilled nursing setting
- Work closely with an interdisciplinary team to ensure optimal patient outcomes
- Complete accurate, timely documentation in Net Health, adhering to facility standards
- Deliver high-quality patient-centered care, adapting plans as patients progress
- Participate in care team meetings and contribute insights for comprehensive care planning
