Schedule:

• Preferred shifts are Sunday–Thursday or Tuesday–Saturday.

Qualifications:

• Current and valid Physical Therapist license in California
• Experience in skilled nursing or rehabilitation settings preferred; mentorship available for candidates with less than one year of experience
• Strong documentation and organizational abilities
• Ability to work flexibly in a team environment
• Willingness to float to a nearby sister facility if needed

Responsibilities:

• Assess, plan, and implement treatment interventions for patients in the skilled nursing setting
• Work closely with an interdisciplinary team to ensure optimal patient outcomes
• Complete accurate, timely documentation in Net Health, adhering to facility standards
• Deliver high-quality patient-centered care, adapting plans as patients progress
• Participate in care team meetings and contribute insights for comprehensive care planning
