Skills & Qualifications:
- Valid state PT license in good standing
- Experience in hospital/outpatient therapy preferred
- Comprehensive knowledge of therapy evaluation and treatment planning
- Ability to work collaboratively and communicate with physicians, patients, and families
- Commitment to professional development and learning
- Familiarity with professional and ethical codes of conduct
Key Responsibilities:
- Evaluate assigned patients, developing and modifying individualized treatment plans based on patient needs, goals, and physician direction
- Delegate and direct discipline-specific therapy services to assigned patients
- Design functional therapy programs to promote effective return to community and daily activities
- Serve as an educator for patients, families, students, and colleagues
- Uphold organizational mission, core values, and compliance policies, maintaining confidentiality and professional standards
- Complete clear, accurate, and timely medical record documentation and goal writing
- Participate in clinic safety initiatives, with an emphasis on safe patient handling and a hazard-free work environment
- Provide student supervision and contribute to a supportive learning atmosphere
- Proactively pursue ongoing professional growth through continuing education and development
