A respected hospital in California is seeking a skilled Physical Therapist to provide high-quality care within home-based therapy services. This contract opportunity is ideal for professionals with strong clinical backgrounds who enjoy working independently and making a direct impact in patient lives through personalized treatment plans.

Schedule:

  • Flexible shifts, primarily Monday to Friday, 8am–5pm
  • Every 3rd weekend rotation
  • Minor holidays; major holidays only as needed

Qualifications:

  • Valid California Physical Therapist license
  • Active BLS certification
  • Minimum of 2 years’ experience in a therapy role, with at least 1 year in PT, OT, or Speech
  • Experience working in a home health setting preferred (not required)
  • Familiarity with Oasis documentation required
  • Proficiency with Cerner EMR platform a plus
  • Ability to adapt to geographically assigned caseloads
  • Willingness to attend a 1.5–2 week orientation

Responsibilities:

  • Provide comprehensive therapy services for patients with a variety of diagnoses, including orthopedic injuries, strokes, joint replacement, and spinal injuries
  • Complete accurate and timely Oasis documentation and utilize Cerner EMR
  • Collaborate with multi-disciplinary teams, including permanent Rehab Aides, OTAs, and PTAs
  • Communicate effectively with patients, families, and care teams to ensure continuity of care
  • Maintain professional appearance: business attire with a white lab coat (jeans not permitted)
