Key Qualifications

• Current California PT license
• BLS certification
• Valid driver’s license, auto insurance, auto registration
• TB (annual), MMR, Varicella, HepB, TDap (within 10 years), and Flu vaccine (annual) compliance
• Successful completion of drug screen and physical exam
• Willingness to travel across assigned and neighboring areas
• Ability to see Covid+ and PUI patients if required

Responsibilities

• Deliver high-quality physical therapy assessment, planning, and interventions in home health settings
• Complete documentation, patient evaluations, and routine visits based on established points-system
• Communicate effectively with patients, physicians, and interdisciplinary care teams
• Adhere to patients' specific care plans and safety guidelines
