Physical Therapist | Pasadena, California

Provide expert physical therapy services within an outpatient setting focused on delivering patient-centered care that supports functional independence and community reintegration. This role involves evaluating assigned patients, designing and updating individualized treatment plans, and overseeing therapy discipline-specific interventions aligned with physician directives and patient goals.

Key Responsibilities:

• Perform comprehensive patient evaluations and develop treatment plans targeting progress toward discharge within an appropriate length of stay.
• Document clinical interventions, goals, assessments, and pertinent communications accurately and promptly following regulatory and clinic guidelines.
• Delegate and coordinate all discipline-specific therapy services for assigned patients to ensure cohesive care delivery.
• Serve as an educator to patients, families, students, and coworkers, fostering a collaborative and supportive learning environment.
• Uphold organizational standards including confidentiality, safety protocols, and professional ethical codes.
• Participate in continuous professional development and comply with state licensing and continuing education requirements.
• Engage in organizational quality improvement initiatives to enhance patient care.

Qualifications & Desired Experience:

• Valid state licensure as a Physical Therapist.
• Experience in hospital or outpatient rehabilitation settings preferred.
• Proficient in clinical evaluation, treatment plan development, and patient documentation.
