Schedule/Hours:

• Full-time, travel assignment (specific schedule to be provided upon offer)

Qualifications:

• Bachelor's degree required; Master’s or Doctorate in Physical Therapy preferred
• Active Physical Therapist license in Massachusetts
• Basic Life Support (BLS) certification preferred
• Previous experience in long-term care or skilled nursing settings is a plus
• Compassionate, patient-focused approach
• Strong interpersonal, communication, and documentation skills
• Proficiency in Microsoft Office and electronic medical records
• Ability to work efficiently within an interdisciplinary team
• Physical ability to lift/carry up to 50 pounds and perform tasks requiring mobility and stamina

Key Responsibilities:

• Evaluate residents and establish individualized therapy plans
• Administer therapeutic procedures under physician’s orders, ensuring patient safety
• Collaborate with caregivers, nurses, and referral sources on care planning
• Monitor progress, updating care plans as needed
• Mentor new staff and provide in-service education
• Maintain accurate documentation and compliance with Medicare and reimbursement regulations
• Ensure department cleanliness and assist with equipment maintenance
• Supervise assistant-level staff, adhering to all relevant standards and guidelines
