This position offers a unique opportunity to work in a Home Health setting with a varied patient population, including orthopedic joint replacements and geriatric patients facing complex comorbidities such as CHF and COPD. Schedule/Hours:

• Day shifts
• No weekends or holiday shifts required

Qualifications:

• Current Physical Therapy license and BLS certification
• Home Health experience required (travelers with less than 2 years’ overall experience will be considered with Home Health expertise)
• Competency with HomeCare HomeBase EMR preferred
• Ability to work independently with a daily average of 5 patient visits
• Valid driver’s license (mileage reimbursed)

Role Responsibilities:

• Conduct start-of-care assessments, especially for orthopedic patients when skilled nursing isn’t ordered
• Manage a flexible caseload within a growing census, supported by an Intake RN, Manager, and nurse aides
• Provide compassionate, evidence-based PT for patients in their homes across most of Tehama County
• Complete documentation using a provided tablet
• Collaborate with leadership on efficient patient load distributions as staff expands
