This position offers a unique opportunity to work in a Home Health setting with a varied patient population, including orthopedic joint replacements and geriatric patients facing complex comorbidities such as CHF and COPD. As you collaborate with an experienced team in an innovative, adaptive care model, you’ll increase your professional versatility through exposure to new technologies (HCHB EMR on a provided tablet), home care delivery methods, and a supportive leadership structure. Schedule/Hours:
- Day shifts
- No weekends or holiday shifts required
Qualifications:
- Current Physical Therapy license and BLS certification
- Home Health experience required (travelers with less than 2 years’ overall experience will be considered with Home Health expertise)
- Competency with HomeCare HomeBase EMR preferred
- Ability to work independently with a daily average of 5 patient visits
- Valid driver’s license (mileage reimbursed)
Role Responsibilities:
- Conduct start-of-care assessments, especially for orthopedic patients when skilled nursing isn’t ordered
- Manage a flexible caseload within a growing census, supported by an Intake RN, Manager, and nurse aides
- Provide compassionate, evidence-based PT for patients in their homes across most of Tehama County
- Complete documentation using a provided tablet
- Collaborate with leadership on efficient patient load distributions as staff expands
