A skilled and adaptable Physical Therapist is needed for a vibrant skilled nursing facility offering both post-acute and outpatient therapy services. This contract opportunity provides the chance to work with a diverse patient population, supporting individuals in regaining mobility, strength, and greater independence. You will thrive in a collaborative, fast-paced setting while gaining valuable experience within an interdisciplinary care team.

Schedule Options:

  • Sunday through Thursday
  • Tuesday through Saturday

Key Qualifications:

  • Current Physical Therapy license in California
  • Experience in skilled nursing facilities preferred (open to those with 0–1 year and mentorship available)
  • Excellent documentation and time management abilities
  • Strong team player with flexibility to adapt as needed
  • Willingness to float occasionally to a nearby sister facility

Primary Responsibilities:

  • Conduct thorough evaluations and develop individualized treatment plans for SNF patients
  • Implement evidence-based therapy interventions to address mobility, pain, and functional challenges
  • Collaborate proactively with interdisciplinary care team members to support patient progress and outcomes
  • Maintain detailed, accurate records in Net Health per facility requirements
