A skilled and adaptable Physical Therapist is needed for a vibrant skilled nursing facility offering both post-acute and outpatient therapy services. Schedule Options:
- Sunday through Thursday
- Tuesday through Saturday
Key Qualifications:
- Current Physical Therapy license in California
- Experience in skilled nursing facilities preferred (open to those with 0–1 year and mentorship available)
- Excellent documentation and time management abilities
- Strong team player with flexibility to adapt as needed
- Willingness to float occasionally to a nearby sister facility
Primary Responsibilities:
- Conduct thorough evaluations and develop individualized treatment plans for SNF patients
- Implement evidence-based therapy interventions to address mobility, pain, and functional challenges
- Collaborate proactively with interdisciplinary care team members to support patient progress and outcomes
- Maintain detailed, accurate records in Net Health per facility requirements
