Key Qualifications:

  • Hold an active California Physical Therapist (PT) license
  • Skilled Nursing Facility (SNF) experience strongly preferred (open to new graduates with mentorship support)
  • Demonstrate strong documentation and time-management skills
  • Embrace flexibility and the ability to adapt to a fast-paced environment
  • Willingness to float between sister facilities within walking distance when needed

Preferred Schedule:

  • Sunday through Thursday or Tuesday through Saturday (preference given to candidates open to a flexible work week)

Primary Responsibilities:

  • Assess, plan, develop, and implement personalized treatment plans for residents with varying functional needs
  • Deliver high-quality hands-on therapy to improve mobility, balance, strength, and independence
  • Complete timely and precise documentation using Net Health in accordance with facility protocols
  • Participate actively in interdisciplinary team meetings to coordinate care and maximize patient progress
  • Adjust therapy goals and approaches in response to patient feedback, changing needs, and care team observations
