Job Type Contract

Provide compassionate, high-quality therapy care to adult patients in a home health setting. This contract position is ideal for experienced Physical Therapists with at least one year of home health experience, who are comfortable working independently while benefiting from a supportive interdisciplinary team. You’ll travel throughout a wide service area, delivering essential therapies to adults with chronic diseases, orthopedic conditions, and post-surgical needs.

Schedule/Hours:

  • 6 patient visits per day
  • No weekend or holiday rotation
  • No on-call requirement
  • Schedule set every 13 weeks

Qualifications:

  • Active state licensure as a Physical Therapist (PT)
  • BLS certification required
  • Valid driver’s license and proof of insurance
  • Minimum of one year of home health experience
  • First-time travelers welcome if experienced in home health

Responsibilities:

  • Provide high-quality, evidence-based PT interventions to adult patients in their homes across a multi-city region
  • Plan, schedule, and confirm daily patient visits with each client the day prior
  • Accurately document assessments, care plans, and progress notes using provided mobile devices and Home Care Home Base system
  • Maintain effective communication with departmental leadership, schedulers, clinical managers, and the clinical team
  • Participate in a collaborative, team-oriented culture focused on quality patient outcomes
