Provide specialized physical therapy services in an outpatient hospital setting, delivering patient-centered care designed to promote functional independence and a safe return to community activities. This role involves comprehensive patient evaluations, development and modification of individualized treatment plans, and delegation and coordination of discipline-specific therapy services.
Key Responsibilities:
- Conduct thorough patient evaluations and utilize clinical reasoning to establish and adjust targeted treatment plans.
- Document patient care, including goal setting, progress notes, quality indicator assessments, letters of medical necessity, and charge entry in a timely and accurate manner.
- Ensure all care is delivered under appropriate physician orders and in compliance with insurance and regulatory guidelines.
- Provide education and support to patients, families, students, and coworkers, fostering a collaborative and informed care environment.
- Participate actively in student clinical education, including supervising and evaluating learners.
- Maintain adherence to professional codes of conduct, ethical standards, and regulatory requirements.
- Contribute to organizational performance improvement initiatives and champion patient and employee safety protocols.
