Schedule/Hours:

• Travel role with weekdays and occasional weekend coverage.
• Flexibility required for occasional travel beyond core assignment area.

What You’ll Bring:

• Current CA Physical Therapist (PT) license required.
• BLS certification (American Heart Association).
• Valid driver's license, car insurance, and automobile registration.
• Annual TB screening, proof of MMR, Varicella, HepB, and TDap vaccinations.
• Annual flu shot, recent drug screen, and physical exam.
• Comfort seeing diverse patient populations, including Covid+ and PUI as needed.
• Reliable transportation for home visits throughout the assigned area.

Key Responsibilities:

• Conduct comprehensive patient evaluations and develop individualized therapy plans.
• Deliver skilled physical therapy in home health settings across assigned territories.
• Complete required documentation, communicate care updates, and coordinate with physicians.
• Meet weekly productivity standards through visit-based point system.
• Maintain compliance with agency and state/federal regulations.
