An exciting opportunity awaits for a skilled Physical Therapist with a strong background in hospital-based therapy to provide exceptional home health services as part of a dedicated multidisciplinary team. This contract position focuses on delivering quality patient-centered care within a supportive environment, ensuring meaningful progress for those rehabilitating at home.

Schedule/Hours:

• Flexible schedule, majority of work hours are Monday to Friday, 8am-5pm
• Weekend shift every third weekend, with coverage for minor and occasional major holidays as needed

Required Qualifications:

• Minimum 2 years of relevant experience as a Physical Therapist
• California PT license in good standing
• Active Basic Life Support (BLS) certification
• Proficiency with OASIS documentation (government-mandated form)
• Experience in home health care settings preferred (PT/OT/Speech), though not required
• Ability to work collaboratively with Rehab Aides, PTAs, OTAs, and other interdisciplinary staff
• Comfortable with Cerner electronic medical records system

Key Responsibilities:

• Provide home-based care for a range of patients, including those with orthopedic conditions, strokes, joint replacements, and spinal injuries
• Complete assessments, develop care plans, and implement therapeutic interventions tailored to individual patient needs
• Travel to assigned geographical regions to deliver safe, effective therapy
• Participate in regular interdisciplinary meetings and follow floor-specific orientation protocols (1.5 to 2 weeks)
• Maintain professional business attire with a white lab coat (no jeans) during visits
