A skilled and motivated Physical Therapist is needed to provide therapy services in a hospital-based setting. This contract role offers the chance to work in a large, vibrant skilled nursing facility focused on patient-centered rehabilitation and recovery.

Schedule:

  • Sunday through Thursday or Tuesday through Saturday preferred
  • May require occasional float to a sister facility within walking distance

Qualifications:

  • Current, active California Physical Therapist license
  • Experience in skilled nursing facilities strongly preferred (mentorship available for candidates with 0-1 years’ experience)
  • Excellent documentation and time-management skills
  • Flexible approach and ability to collaborate within a multidisciplinary team

Responsibilities:

  • Assess residents and develop individualized treatment plans to improve mobility, strength, and daily living skills
  • Deliver physical therapy services and modify interventions based on patient progress
  • Collaborate closely with the interdisciplinary care team to support holistic care
  • Accurately and efficiently document care using Net Health in accordance with facility standards
  • Contribute to a supportive environment focused on maximizing each patient’s independence
