A skilled physical therapist is needed to provide specialized care in a critical illness and rehabilitation setting. This contract opportunity offers the chance to support patient recovery through comprehensive evaluations, tailored treatment plans, and collaborative care aimed at optimizing discharge outcomes. Responsibilities:
- Perform comprehensive patient evaluations to develop effective, goal-oriented treatment plans.
- Collaborate with interdisciplinary teams to overcome barriers to discharge, including equipment management and patient/family education.
- Manage individual, concurrent, and group therapy sessions efficiently based on patient needs.
- Maintain accurate and timely medical documentation, including daily notes, goal updates, assessments, and discharge summaries.
- Ensure all care is delivered under valid physician orders and insurance approvals.
- Participate in the supervision and mentoring of students as assigned, fostering a positive learning environment.
- Develop and pursue individualized professional growth plans, engaging in continuing education and professional activities.
- Actively engage in patient and professional advocacy, including community events and health initiatives.
- Comply with state practice acts, maintain licensure, and uphold ethical standards.
- Support organizational performance improvement initiatives by providing feedback and following action plans.
Desired Experience & Qualifications:
- Valid physical therapist licensure in accordance with the relevant state practice act.
- Demonstrated competence in hospital-based physical therapy and rehabilitation.
- Strong clinical reasoning skills and ability to develop patient-centered treatment plans.
- Experience collaborating within interdisciplinary healthcare teams.
- Commitment to maintaining safety standards and following hospital policies.
- Ability to work flexible caseloads and adapt to dynamic clinical environments.
