Providing specialized physical therapy care in an acute inpatient setting is crucial to patient recovery and rehabilitation. This role focuses on delivering therapeutic physical therapy services within acute rehab as the primary coverage area, with secondary experience in ICF/SNF rehab settings. The position requires a candidate skilled in evaluating and re-evaluating residents or patients, designing and implementing ongoing physical therapy programs as prescribed by the attending physician or consulting specialist, and offering expert consulting physical therapy services.
Responsibilities include:
- Conduct thorough evaluations and re-evaluations of assigned patients.
- Develop and execute individualized therapeutic physical therapy plans.
- Collaborate closely with healthcare teams to optimize patient outcomes.
- Be adaptable to variable shifts, including weekends and holidays, based on hospital operations.
- Utilize EMR systems, specifically Meditech, for documentation and care coordination.
Qualifications and Desired Experience:
- At least 6 months of work experience in one or more of the following adult care settings: Acute care, rehab hospital, or skilled nursing facility.
- Current BLS certification.
- Active State of Hawaii Physical Therapy license or ability to obtain prior to assignment start date (application for license must occur upon offer acceptance).
- Flexibility to work variable schedules, including Mon-Fri, Tues-Sat, or Sun-Thurs shifts (8:30 am to 5:00 pm).
- Ability to arrive on-site a day early for employee health appointment prior to start.
