Schedule/Hours:
- Preference for Sunday through Thursday or Tuesday through Saturday shifts
- May require occasional floating to a nearby sister facility within walking distance
Qualifications:
- Current and active California Physical Therapist license required
- Previous experience in a skilled nursing facility setting is preferred; mentorship is available for those with up to one year of experience
- Strong documentation and time-management abilities
- Flexible, team-oriented approach with a willingness to collaborate across disciplines
Responsibilities:
- Conduct thorough evaluations and deliver individualized treatment plans for skilled nursing facility patients
- Work closely with an interdisciplinary care team to ensure optimal, coordinated patient care
- Complete timely and accurate documentation in Net Health, adhering to established facility standards and protocols
- Adjust therapeutic interventions as needed to meet evolving patient goals and needs
