Schedule/Hours:

• May require occasional floating to a nearby sister facility within walking distance

Qualifications:

• Current and active California Physical Therapist license required
• Previous experience in a skilled nursing facility setting is preferred; mentorship is available for those with up to one year of experience
• Strong documentation and time-management abilities
• Flexible, team-oriented approach with a willingness to collaborate across disciplines

Responsibilities:

• Conduct thorough evaluations and deliver individualized treatment plans for skilled nursing facility patients
• Work closely with an interdisciplinary care team to ensure optimal, coordinated patient care
• Complete timely and accurate documentation in Net Health, adhering to established facility standards and protocols
• Adjust therapeutic interventions as needed to meet evolving patient goals and needs
