Physical Therapist - Watsonville, California

Seeking a skilled and adaptable physical therapist to provide exceptional therapy services within a dynamic skilled nursing facility setting. This contract opportunity offers the chance to work with a diverse patient population, utilizing evidence-based practices to help individuals regain mobility, strength, and independence.

Schedule Options:

• Sunday through Thursday
• Tuesday through Saturday

Qualifications:

• Current and active California Physical Therapist license
• Prior experience in a skilled nursing facility preferred (mentorship provided for those with 0-1 years of experience)
• Strong proficiency in patient documentation and time management
• Ability to work collaboratively within an interdisciplinary team
• Flexibility to float to a nearby sister facility as needed

Key Responsibilities:

• Evaluate and treat patients requiring physical therapy interventions in a skilled nursing facility environment
• Develop, implement, and continuously update individualized care plans to support patient rehabilitation goals
• Participate actively in care team meetings to ensure quality patient outcomes
• Accurately complete all documentation using Net Health according to facility standards
