Schedule:

• Preference for Sunday–Thursday or Tuesday–Saturday shifts.

Qualifications:

• Active California Physical Therapist (PT) license.
• Previous experience in skilled nursing facilities preferred; mentorship provided for those with 0–1 year of experience.
• Proficiency in documentation and effective time management.
• Positive, flexible approach with the ability to work collaboratively within a multidisciplinary team.
• Willingness to float to a nearby sister facility if required.

Key Responsibilities:

• Conduct thorough evaluations and implement comprehensive treatment plans for skilled nursing facility residents.
• Deliver patient-centered therapeutic interventions to facilitate progress toward mobility and independence goals.
• Work closely with the interdisciplinary care team to coordinate patient care strategies.
• Maintain timely, accurate documentation using Net Health per facility standards.
