Skilled Nursing Facility Physical Therapist | Bolivar, Missouri

A skilled Physical Therapist opportunity is available for a contract position located in Bolivar, MO, within a school-based setting. This role focuses on evaluating and rehabilitating patients with physical functioning disorders and wound care, delivering care in accordance with regulatory and clinical standards.

Key Responsibilities:

• Conduct comprehensive screening, evaluation, and rehabilitation planning for patients with physical impairments and wounds.
• Utilize standardized assessments and evidence-based practices to guide clinical interventions.
• Implement various rehabilitation procedures such as manual therapy, ambulation training, therapeutic exercises, use of assistive devices, and wound care.
• Monitor patient progress and modify treatment plans accordingly; evaluate treatment outcomes and coordinate referrals as needed.
• Provide consultation and counseling to patients, families, caregivers, and service providers regarding physical disorders.
• Maintain clear, accurate documentation in electronic medical records following all regulatory and company policies.
• Collaborate effectively with supervisors and interdisciplinary healthcare team members to address patient needs and treatment goals.
• Prepare comprehensive discharge summaries aligned with clinical and regulatory requirements.
• Participate in patient care conferences, utilization reviews, facility meetings, and ongoing education including trainings and in-services.
• Supervise and support physical therapist assistants, rehabilitation aides, and students.
• Ensure compliance with professional standards, including adherence to the American Physical Therapy Association (APTA) and state practice acts.

Qualifications & Desired Experience:

• Licensed Physical Therapist with relevant clinical experience in rehabilitation and wound care.
• Strong knowledge of assessment tools and evidence-based therapeutic techniques.
