1:1 Pediatric Skilled Nursing Facility Physical Therapist - Hamden, Connecticut

In this contract position, you will provide valuable therapy services for a student in a supportive school environment. Qualifications and Desired Experience:

• Active Physical Therapist license
• Experience in pediatric physical therapy, preferably in a school or educational setting
• Strong skills in providing one-on-one interventions with children
• Familiarity with school-based documentation and collaboration practices
• Effective oral and written communication with families and educational teams
• Ability to adapt therapy plans to meet individual student needs

Responsibilities:

• Deliver direct physical therapy to a student for 30 minutes each week
• Conduct consultations and provide recommendations for 45 minutes per trimester

• Track progress, maintain accurate records, and communicate updates to relevant stakeholders
