Licensed Practical Nurse (LPN) in Castorland, NY

Deliver exceptional patient-centered care in a supportive environment specializing in long term care services. Experienced LPNs and RNs are sought for a contract role where your clinical skills will help improve the lives of residents needing ongoing medical and daily living support.

Key Responsibilities:

• Assess, plan, implement, and evaluate nursing care for residents with chronic health needs
• Administer prescribed medications and treatments, ensuring safe and effective delivery
• Collaborate with physicians, interdisciplinary teams, and family members to provide coordinated care
• Monitor and document vital signs, patient progress, and any changes in condition
• Support residents with daily living activities and encourage independence where possible
• Adhere to infection control protocols and safety standards in a long term care setting
• Provide compassionate emotional support to residents and families
• Supervise and mentor aides or other support staff as assigned

Qualifications:

• Current and unrestricted LPN or RN license
• Strong clinical assessment and critical thinking skills
• Previous experience in Long Term Care or similar environments preferred
• Familiarity with electronic medical record systems
• Excellent communication and organizational abilities
• CPR certification (current)
• Commitment to upholding high standards of patient care and professionalism
