Qualifications & Desired Experience:

  • Active and unencumbered LPN license.
  • Prior experience working with patients requiring tracheostomy care.
  • Strong assessment, observation, and documentation skills.
  • Proven ability to administer medications and treatments according to care plans.
  • Clear communication skills and a positive, professional attitude.
  • CPR/BLS certification.
  • Flexibility to adapt to varying patient needs and healthcare settings.

Key Responsibilities:

  • Deliver skilled nursing care to patients, with a focus on those requiring trach management.
  • Monitor and assess patient conditions, reporting significant findings to the supervising RN or physician.
  • Administer prescribed medications and treatments, ensuring patient safety.
  • Maintain accurate documentation of care provided and communicate effectively with the healthcare team.
  • Educate patients and families on respiratory care, trach management, and health maintenance as needed.
  • Adhere to all facility protocols and regulatory standards for infection control and patient safety.
