Deliver comprehensive care in a skilled nursing environment as an LPN specializing in rehabilitative and continuing care services. This contracted role requires experienced clinical judgment, adaptability across facilities, and dedication to the recovery and well-being of residents with a variety of acute and chronic conditions.
Schedule/Hours:
- 8-hour night shifts; 11:00pm – 7:00am, Monday through Friday
- 40 hours per week
- Every other weekend required
- 75% of holidays included in rotation
- No on-call or call back
- Floating within a dedicated rehab region (Cleveland Pines, Huntersville Oaks, Jesse Helms, Sardis Oaks, Stanly Manor)
Qualifications/Desired Skills:
- Active LPN license and current BLS certification
- At least 18 months’ experience in skilled nursing, long-term care, or rehab settings
- Proficient in trach management, IV starts and management, wound care, dressing changes, foley care, and tube feedings
- Confident with EPIC, Alaris IV pumps, and Omnicell medication dispensing
- Willing and able to float to various regional facilities within scope of practice
- Minimum permanent residence radius of 50 miles from assigned home unit facility
Key Responsibilities:
- Deliver safe, quality nursing care to residents requiring both short-term rehabilitation and long-term care
- Accurately perform medication administration, monitor complex conditions, and manage IVs/tube feedings
- Collaborate with interdisciplinary teams including therapies and case management
- Complete documentation using modern EMR (EPIC)
- Adapt to shifting needs and support consistent coverage by floating as needed within the designated rehab region
