Contribute dedicated patient care in a skilled nursing environment, utilizing your experience as a Licensed Practical Nurse (LPN). This contract position involves providing comprehensive services to patients requiring both short-term rehabilitation and long-term care support. You will be instrumental in supporting a diverse patient population, including those recovering from stroke, amputation, neurological disorders, and joint replacement procedures.

Schedule & Shift Details:

  • 8-hour night shifts, 40 hours per week
  • Hours: 11:00 PM – 7:00 AM, Monday through Friday
  • Every other weekend rotation
  • No on-call or call back responsibilities
  • Holiday rotations: travelers are scheduled for 75% of holidays

Qualifications:

  • Current LPN license with BLS certification
  • Minimum of 18 months’ experience in a skilled nursing, long-term care, or rehab facility
  • Proficiency in trach management (refresher provided during orientation), foley care, wound care, IV starts/management, dressing changes, and tube feedings
  • Previous experience with EPIC EMR, Alaris IV pumps, and Omnicell medication dispensing preferred
  • Flexibility to float within the designated regional facilities, as needed
  • Must maintain a permanent address at least 50 miles from the facility

Responsibilities:

  • Deliver high-quality nursing care to patients with diverse diagnoses and rehabilitation needs
  • Perform admissions (supported by admin nurse), manage daily documentation, and execute physician-ordered treatment plans
  • Support multidisciplinary care with nursing leadership, case management, therapies, and ancillary staff
  • Maintain effective communication and ensure consistent care transitions while floating between facilities during shift, as required
