Seeking a dedicated LPN with robust experience in skilled nursing settings for a contract role focused on continuing care. This opportunity is ideal for those who thrive in fast-paced environments and are comfortable with regional floating among specialized rehabilitation facilities.
Schedule & Shift Information
- 8-hour night shifts, 40 hours per week
- Monday to Friday, 11:00 PM – 7:00 AM
- Every other weekend required
- Expect to work 75% of holidays
- No on-call or call-back requirements
Key Qualifications
- Active LPN license and current BLS certification required
- At least 18 months of recent SNF or LTC rehab facility experience
- Experience with trach management (refresher available in orientation), wound care, foley management, IV initiation and maintenance, dressing changes, and tube feeding
- Strong familiarity with EPIC EMR, Alaris IV pumps, and Omnicell med dispensing preferred
- Ability to float as needed across multiple sites within the rehabilitation region
- Must reside 50+ miles from the home unit per travel radius policy
Responsibilities
- Deliver patient-centered care to both short-term rehab and long-term residents coping with recovery from strokes, amputations, neurological disorders, and post-joint replacement procedures
- Collaborate with RNs, CNAs, therapists, case management, and nursing leadership to maintain optimal care standards
- Complete basic nursing duties, including medication administration, monitoring, and documentation in EPIC
- Support department needs by floating within the region when necessary (travel time between sites is paid)
- Handle approximately 1-2 admissions daily as facilitated by administrative nursing staff
