This contract opportunity is perfect for experienced Licensed Practical Nurses (LPNs) skilled in both short-term rehabilitation and long-term care. Work within a dedicated team, utilizing state-of-the-art technology and modern care methods, while making a genuine impact on patients recovering from illnesses, injuries, or surgeries such as stroke, joint replacement, and neurological disorders.
Schedule/Hours:
- 8-hour night shifts: 11:00 PM – 7:00 AM, Monday through Friday
- 40 hours per week
- Every other weekend required
- Minimal holiday commitment (75% of holidays)
- No on-call or call-back requirements
Key Qualifications:
- Active LPN license and current BLS certification required
- At least 18 months of experience in a Skilled Nursing Facility or Long-Term Care rehabilitation setting
- Familiarity with trach management (refresher provided), foley care, wound care, IV initiation/maintenance, tube feedings, and dressing changes
- Proficiency with EPIC EMR, Alaris IV pumps, and Omnicell medication dispensing systems
- Ability and willingness to float between facilities in the Rehab Region as needed
- Permanent address located at least 50 miles from the home facility
- Preferred: prior travel experience in similar settings
Responsibilities:
- Deliver top-quality nursing care to a diverse long-term and short-term patient population
- Collaborate with nursing leadership, therapy, care management, and admissions teams
- Maintain accurate patient records and ensure proper documentation using EPIC EMR
- Adhere to facility’s protocols for cleanliness, safety, and infection prevention
- Manage medication administration and essential nursing tasks as outlined by your scope of practice
