A skilled nursing facility is seeking an experienced LPN to support continuing care for patients requiring both short-term rehabilitation and long-term care. This contract role focuses on delivering nursing care overnight as part of a dedicated team serving a diverse patient population in a dynamic setting.

Schedule & Hours:

  • 8-hour night shifts, Monday through Friday, 11:00 PM to 7:00 AM
  • 40 hours per week
  • Every other weekend required
  • Expectation to work 75% of holidays
  • No on-call or call back required
  • Floating within the region may be required (travel compensated during working hours)

Desired Qualifications:

  • Current and valid LPN license
  • Active BLS certification
  • Minimum 18 months of experience in a skilled nursing facility or long-term care/rehab setting
  • Proficiency in trach management (refresher provided), foley care, wound care, dressing changes, IV starts/management, and tube feedings
  • Comfortable with floating among regional facilities within your scope of practice
  • Familiarity with EPIC EMR, Alaris IV pumps, and Omnicell med dispensing systems
  • First-time travelers not preferred

Key Responsibilities:

  • Deliver direct nursing care for patients recovering from illness, injury, s/p joint replacement, stroke, neurological issues, and amputation (no memory care)
  • Collaborate with a multi-disciplinary team including nursing leadership, therapies, and case management
  • Participate in admissions and deliver 1-2 per day as needed (supported by admin nurse)
  • Maintain patient ratios standard to facility needs (1:26 on nights)
  • Complete all clinical documentation accurately and timely
  • Float within department and other region facilities as required
