Qualifications:
- Active and unencumbered LPN license in good standing
- Proven experience and proficiency with tracheostomy care
- Strong clinical assessment and critical-thinking abilities
- Ability to effectively communicate with patients, families, and interdisciplinary teams
- Up-to-date BLS certification required
- Previous experience in long-term care or a hospital setting is a plus
Key Responsibilities:
- Deliver direct nursing care to patients, including medication administration, monitoring vital signs, and assisting with activities of daily living
- Provide specialized tracheostomy care, including suctioning and monitoring, in accordance with physician orders and facility protocols
- Document all patient interactions and maintain accurate records in compliance with state and facility guidelines
- Collaborate with physicians, RNs, and allied health professionals to ensure coordinated and effective patient care
- Educate patients and families about treatments, procedures, and aftercare
- Uphold infection control and safety standards at all times
