Qualifications:
- Current and unencumbered RN license
- Experience in rehabilitation or long-term care strongly preferred
- Excellent assessment and clinical skills
- Strong communication and interpersonal abilities
- Ability to work independently and as part of an interdisciplinary team
- CPR/BLS certification required
Responsibilities:
- Conduct comprehensive nursing assessments for rehabilitation and long-term care residents
- Develop, implement, and adjust individualized care plans based on patient needs and responses
- Administer medications, treatments, and therapies in accordance with physician orders and facility protocols
- Collaborate with therapists, physicians, and other healthcare professionals to ensure holistic patient care
- Document observations, interventions, and outcomes accurately and promptly in medical records
- Educate residents and families on care processes, safety, and rehabilitation goals
- Monitor patients for changes in condition and respond appropriately to emergent situations
- Foster a positive, compassionate, and encouraging environment for both residents and colleagues
