Key Qualifications & Experience:

• Active, unencumbered RN license
• Prior experience in rehabilitation or long-term care settings strongly preferred
• Strong assessment, critical thinking, and care-planning skills
• Compassionate approach with patients and families
• Ability to multitask and adapt in fast-paced situations
• Proficiency with electronic health records (EHR)

Primary Responsibilities:

• Deliver direct patient care in accordance with individualized treatment plans
• Collaborate with interdisciplinary team members to develop and update care strategies
• Administer medications and perform regular health status assessments
• Document all care provided following legal and facility guidelines
• Educate patients and families regarding recovery and long-term health management
• Monitor vital signs and respond to changes in patient condition promptly
• Ensure infection control standards are maintained
