Key Responsibilities:
- Complete comprehensive nursing assessments upon admission and develop individualized care plans
- Collaborate with patients, families, and interdisciplinary teams to set and achieve rehabilitation goals
- Participate actively in all phases of the patient rehabilitation process
- Maintain detailed, timely medical documentation and communicate changes in patient status
- Initiate discharge planning and education from day one
- Provide leadership, direction, and supervision to ancillary staff
- Ensure personal and environmental safety, adhering to all safety and infection control guidelines
