Key Responsibilities:

  • Perform thorough nursing assessments upon patient admission, forming the basis for individualized care plans.
  • Drive patient rehabilitation programs through direct, compassionate care and evidence-based nursing processes.
  • Identify teaching needs and partner with families, colleagues, and interdisciplinary teams to optimize patient outcomes.
  • Accurately document all assessments, interventions, and patient responses in compliance with facility policy.
  • Delegate and supervise ancillary staff, provide direction, and support effective problem-solving and team building.
  • Take proactive steps in discharge planning and patient/family education from admission onward.
  • Uphold strict safety protocols in patient handling, infectious disease control, and environmental precautions.
  • Promote a positive, efficient work environment and maintain open communication channels across care teams.
  • Seek opportunities for ongoing learning and professional development.

Qualifications:

  • Active RN license and BLS certification.
  • Prior experience in rehabilitation, long-term care, or related nursing settings.
  • Strong organizational, assessment, and communication skills.
